If you’re looking for help paying for assisted living or a group home in Minnesota, the MnCHOICES assessment is where the process starts.
It’s a free evaluation that helps determine what kind of care your loved one needs โ and which programs can help pay for it. Think of it as the key that opens the door to Minnesota’s long-term care programs.
Step-by-Step โ How to Get Started
Step 1: Contact your county Call your local county human services office and ask for a MnCHOICES assessment. Be specific that you are requesting a MnCHOICES assessment so they direct you to the proper person to get it scheduled (The Arc Minnesota)

Step 2: Prepare for the assessment Think about what is important to you, your goals and hopes, and what you need help with. Make a list of health care providers, diagnoses, and medications (Washington County). If you have existing medical records or Social Security disability documentation, have those ready too.
Step 3: Complete the assessment A certified assessor will call you to schedule the assessment, which will take place in your home or another location of your choice. Family, friends, or other supports are welcome to join.
Step 4: Review your results After the assessment, the certified assessor will contact you to let you know which programs you may be eligible for and explain next steps. If you qualify for waiver services, a case manager will be assigned to help you access care.
