Who Qualifies for Customized Living in Minnesota?

Your loved one may qualify if they are enrolled in one of Minnesota’s Medicaid waiver programs. Here’s a quick overview:

How to Get Started With Customized Living Home Service in Minnesota

In Minnesota, getting started with Customized Living services involves a specific process, especially since these services are typically funded through state-sponsored waiver programs. Because “Customized Living” is a specific designation under the Minnesota Department of Human Services (DHS), the path usually begins with a county-level assessment.

Here is the step-by-step guide to starting services.

1. Determine Your Eligibility

To qualify for Customized Living services, an individual must generally be eligible for one of the following Minnesota waivers:

  • Elderly Waiver (EW)

  • Brain Injury (BI) Waiver

In Minnesota, Assisted Living refers to the licensed building and the base level of housing and services provided. Customized Living (CL) is a specific service package funded by Minnesota Medicaid waivers (CADI or EW). It allows you to receive a highly tailored set of health and support services while living in a community setting. 

To have the state pay for Customized Living, you must be enrolled in one of these programs:

  • CADI Waiver (Community Access for Disability Inclusion)

As of July 1, 2025, Minnesota implemented the Residential Support Service Criteria (RSSC). This means new applicants for Customized Living must demonstrate “complex medical or behavioral health needs” to be approved. If you were already receiving services before this date, you are generally “grandfathered” in, but new applicants will undergo a stricter MnCHOICES assessment to ensure they need this specific level of care.  Call us at 763-299-0025 for immediately evaluation.  

Technically, no. Customized Living is designed for individuals living in a “registered housing with services” establishment or an assisted living facility. If you live in a private home you own or rent independently, you would likely look into Individualized Home Supports (IHS) or Personal Care Assistance (PCA). 

No. Medicaid waivers in Minnesota cover the services (nursing oversight, help with bathing, meal prep, etc.), but they do not cover “room and board” (rent).
